The goal of this program is to provide individuals, regardless of their disability, an opportunity to choose community-based living as a viable alternative to nursing home residency.
The program is open to residents of a nursing facility who are receiving Medicaid and request to either return to their own home, or find a less restrictive, more independent environment. Each situation is reviewed on an individual basis.
- A meeting will be arranged to begin the planning process and to start identifying needed resources. Other people may be involved as the individual desires.
- A Person Centered Plan will be conducted during the beginning stages of this process. It is very important for the individual to be actively involved. The transition process can take several months to complete before a person can move back into the community. This is important to ensure a safe and successful return.
We will assist people with returning to their own home, moving in with family members, friends or finding other independent living arrangements.
- We will provide assistance in finding affordable housing and help those who qualify apply for vouchers, if they are available in their community.
- A thorough assessment can be done if there are housing barriers or services are needed. We can assist in identifying helpful modifications.
- We will provide assistance in locating the necessary supports and services to remain in an independent living setting for as long as possible. Typical support services may include: advocacy, transportation, financial assistance, care management, personal care, housekeeping and social needs.
